Over the past five years, the Department of Veterans Affairs has declared more than 4,000 living veterans to be dead, canceling their benefits. The VA has changed its procedures to address the errors, but it has caused major problems for the some veterans. Wall Street Journal reporter Ben Kesling tells Here & Now’s Meghna Chakrabarti how this happened.
